  • Erie County residents can stop by and receive counseling, referral and complete a short health assessment. 

  • If found eligible after the assessment, then you will be given nicotine patches, lozenges and/or gum (while supplies last).  The individual that will be using the products must be present for the assessment. As our staff sometimes gets called out of the office it is best to call first to ensure that there is someone will be available to meet with you and to see if supplies are available.
